What information do I need to include in a mailing matrix?

More details to this question:
When I am typing my mailing matrix, what info do I need to include besides name and address of the creditor? Do I list the amount owed or account numbers?

The mailing matrix includes ONLY the creditor's name and address and if applicable, an attention line.

No but the last four digits of the acct number can help them find and index the acct properly.

If you can't even figure out how to do the matrix then I strongly recommend you get an attorney because it gets a lot more complicated and dangerous as to what you put in the petition, schedules, statement of financial affairs, etc. and how you put it.
