What kind of case is this?

More details to this question:
In 2012 a psycologist I saw for a year put in my medical file (for all medical professionals to see) a diagnosis of Pedophilia! I have my complete file from this drs office, nowhere in this file is any indication of any such thing, it is a malicious lie made by this person, Im assuming because after a year I told her that her methods werent working for me and parted ways. This diagnosis was dated the very day she met me, the notes from that day are simply intake notes, getting my personal info, etc. This has been in my record for 6 years without my knowledge! Is this a libel case, a malpractice case....please help

In theory this would be a malpratice case if negligent and likely some other form a case if done intentionally as you allege. Bottom line is such a case would rarely be a contingent fee case and you would need to willing to spend money out of pocket to address this issue effectively for a variety of reasons.
