Dentist forged my signature on loan

More details to this question:
Went to a dentist for a broken tooth he decided he wanted me to have a root canal,I told him that I did not want to have it done. Dentist applied for loan with care credit forged my signature recieved $ 7000 and now I am responsible for this money. I never approved, nor requested a loan let alone have him or his office forge my signature. What steps should I take. His office refuses to listen and correct problem
