How do I find an abandonment form to remove one party from my contract

More details to this question:
We purchased a contract membership with a Travel Agency. My partner on the contract has since moved away and I can no longer contact this person. I have recently upgraded the contract with more benefits, but can not complete this upgrade without a legal abandonment form releasing the Travel Agency from all claims from my previous partner on the contract. Can you help me?

Unless the other side agrees, there is no legal right to drop this other party.
