More details to this question:
My son turns 18 in September and I need to obtain a POA for him. He is mentally and physically disabled and not able to care or make decisons for himself.

If your son is truly mentally and physically disabled, you should consider more than merely power of attorney over your son but the establishment of a formal guardianship.
