Do I still need to get the interlock system?

More details to this question:
I got a DUI in 2008. I served a year suspension of not driving. Then on 7-22-2009 I was eligible to get a interlock system for a year. I ended up not getting one and not driving for that year. It is now 2014 do I still have to get an interlock system to get my license?
