Is a consent to search form a legal document?

More details to this question:
Would it be considered a contract if juvenile is involved and their legal guardian signs where the juvenile is supposed to sign and the juvenile signs where their legal guardian is supposed to? Does that make the consent search form invalid?

It is a legal document. It is not a contract. A mere switch of signatures on the wrong lines will invalidate neither a contract or a consent form.
