What do I need to reinstate after I got a DUI?

More details to this question:
I had a DUI in 2009 and the court date was 2010. I had two extensions that were completed on 2011. I never got them unsuspected. I drove one time and I had an accident. DUI was cleared but warrant exist for failure to appear. I was in the hospital for 6 weeks. I was forced to go to out of state for health reasons.

I can't really follow your question, you should probably get your driver's record from PennDOT and review it with a lawyer. Generally, you need to serve your suspensions, then fill out a restoration form with PennDot and send the $25 fee in. In order for your suspension to even begin to run, you need to submit your driver's license to them.
