I had my divorce in NY the domestic relations order was done incorrectly and contradicts the settlement pages. I need to file papers in divorce court to fix this myself( It is a long story as to why I am doing it myself-I cant afford a pension attorney what form to do I file to fix the domestic order regarding the pension? Thank you

There is no "form" for this sort of thing. You need either a stipulation or a motion, depending upon whether your ex agrees with the proposed change - and you need a properly drafted QDRO.
