Uncontested Divorce in New York

More details to this question:
A friend of mine recently told his wife he is going to start the divorce process. They were married in 1996 and she left him in 2005. They have  court ordered child support (which he pays each week) and a set visitation schedule. She has informed him she will not contest the divorce as long as he pays the fees.  I am helping him out and have prepared: automatic orders, notice concerning continuation of health insurance, verified complaint, action for divorce, and the affidavit of defendant in action for divorce.  Are there any additional forms required to start the process? If so, what forms are needed? Also, what additional forms will be needed in order to finalize the process? Thank you in advance.

Your friend should either have a lawyer prepare the papers, or if he really doesn't want to hire an attorney, all the Supreme Courts have a free court help office where he can go and get help in preparing these documents (and this will help make sure that the papers are approved by the judge who will sign the divorce).
