Transfer House to Caregiver?

More details to this question:
I am in Delaware, and I heard that a house can be transferred to the name of a family/Caregiver if they have lived there for more than two years to prevent the elder having to move to assisted living. I believe this transfer would keep the house out of any financial reckoning should the elder need Medicaid. But the elder's house in a Residuary Trust. Can this sort of transfer still occur from a Residuary Trust?

there is something like that in PA but you need to ask a DE lawyer.
