Is there a separate release form for all family members?

More details to this question:
When signing a model release, does the signature of one parent in the family represent the release for all immediate family members? Or would individual releases be needed for all family members? This is for a photography model release.

It depends on how the agreement is written but generally individual releases are needed for each person.

A release signed by the legal guardian of a minor (i.e under 18 years old) is necessary when conducting modeling shoots. That release will likely only apply to that individual minor model, unless the release says differently. In other words, you probably need a release for the other child.

Normally a separate release is signed by each adult family member from whom a release is needed (for example, all adult family members who are in the photos). Also, if there are children in the photo who are not of legal age, then a parent or legal guardian of each such child must sign a release as well (on behalf of the child). There may be particular circumstances relating to your specific situation, or relating to the particular State in which you live, which might dictate a different answer, but that is the general rule. You should consult with your attorney regarding the particulars of your situation, though.
