It could be some type of fraud. You need to contact the administrator of the plan to see what they can do for you.

You need to contact the administrator of the 401k plan immediately, and if answers are unsatisfactory, obtain yourself a litigation lawyer to possibly sue the plan administrator for mismanagement.

Not unless someone forged you name and produced documents. You should contact plan administrator to see how it was done and do whatever they suggest. You may have to file an action, both criminal and civil, against whoever did it, and file some kind of action against plan administrator if they released it to an unauthorized person.

Contact the plan administrator immediately and find out what happened. If there is fraud involved or something the plan administrator did wrong, you may be able to sue, but there is just not enough information right know to know how to fix the issue.
