How do I add my last name (maternal mother with full custody) to my children's last name?

I am not trying to remove their paternal father’s name, I just want mine added. Can someone guide me in the right direction? This would mean everything to my children and even more to me. Their father is now MIA after 15 years together (never married) and we are all tired of not having the same last name, especially with him out of the picture. Any support you can offer would mean so much. Thanks in advance.

You can simply file a Petition for Name Change and remove his name if you want. The issue will be service of the Petition on him.
