How to ask for a hearing to relocate with minor child

More details to this question:
I am the custodial parent of a minor child. I share legal custody with the father. I have been offered a better job out of state. I have asked the father for permission but have gotten no answer. He does pay child support but does not do visits which he could if he wanted to but does not. How do I ask for a hearing for removal.

You need to file a Complaint for Massachusetts and request that you can remove the child. You will need to show that it is a real advantage.
