I need some type guardianship for my grandson

More details to this question:
My daughter has a drug problem and sometimes is gone for days/weeks. I do not want to file for custody due to a sitaution with the natural dad (he has not ever seen the child since birth) I just need to get some document that allows me to take him to doctor if needed and enroll him in school
