QUESTION

Looking for Order for Default form

More details to this question:
I've conducted diligent search following news paper publication and did not find my husband. I sent a "Motion for Default" to the Judge but I'm told by his secretary that I also need a form "Order for Default". I've searched all over the internet for that form but can't find it. Where can I get that form?
