More details to this question:
Hi, We are from Indian origin. My wife name is Rati Agarwal and we blessed baby in month of september. My wife's name before marriage was Renu Agarwal. By Mistake we have written mother's name on our baby's birth certificate as Rati Renu instead of Rati Agarwal. Now we are trying to correction mother's name as Rati Agarwal on our baby's birth certificate. Vital department is asking court order as name change. But we need to correct the name not change the name. So could you please suggest ? If we go court then what type documents will be required?

A petition to change the name needs to be filed in District Court. The Court will hear the matter in a short hearing and approve the name change. The Court order will be sent to the Bureau of Vital Statistics to get the new birth certificate issued.
