Visitation agreement

More details to this question:
My daughter lives in Tuscon with her grandmother under a guardianship. The guardianship was transferred from Illinois. This transfer was allowed because we all signed an agreement that was entered into the court that established my visitation rights. I get summer and a rotating holiday between christmas and thanksgiving. Now, the grandmother is not abiding by the agreement. I spent all I had to get this visitation in place and now I need to know how to file a motion from Utah in order to make the court enforce the agreement that was entered under this case.

There are a number of questions that must be answered first, such as where original custody orders are, the circumstances for the guardianship, the grandmother's contacts with Utah, and the child's contacts with Utah.
