HIPAA question

More details to this question:
I still see some questionnaires- job applications, government permits- still ask for disease information, is this allowed under HIPAA laws?

HIPAA prevents a medical provider from revealing your personal medical records without your consent.  The job application is asking you to give consent.  Companies can require drug testing, and request medical records.
