Permanent Disability Question

Good Afternoon - I was helping my sister fill out an application for permanent disability in the state of New Jersey, and question 4A asks if she is currently working, which she is. She has a whole host of health issues and has since she was born. Her main problem right now are dizzy spells, which cause her to fall at times and come close to passing out. They've been running all kinds of tests on her and have yet to find a reason as to what's causing the problem. Her one doctor came right out and told he that he does not want her driving or working. The one thing that might make her claim look suspicious, is that the company she works for is closing in April. What should we do...fill her paperwork out now while she IS working, or wait for her to be out of work? She DOES have an entire file of medical paperwork to back up her claim. The only reason she keeps working is that she has 2 children she's raising by herself. I appreciate any help you can give for her. Thank you
