Dear Sir, I am from India and having Indian passport. I came USA 4 months back on L1B Visa and its valid till 2018. I have query for dependent visa for My wife and My daughter. My wife has Pakistani passport and currently she is in India. My 3.5 years old daughter have Indian passport. So My query is - I have marriage certificate. Last name in Marriage certificate and passport of my wife are different. And Actual correct last name is in Marriage certificate. So, Which I need to mention in DS-160 ? Will this create any problem at visa interview ?

Reveal all the information on the forms, just as you stated in your question.

You need to enter her legal name on the DS-160. You should disclose all other names she has used where indicated on the form. She has the burden to establish her identity and relationship to you.
