Your mother should fill out form I-102 application for a replacement I-94 card. She should attach as much evidence that she can pertaining to her entry including a detailed statement, and any documents that she may have kept including flight ticket, boarding pass, baggage claim receipts, etc. The I-102 is to be submitted to the U.S.C.I.S. lockboxes in either Dallas or Phoenix depending upon your mother’s residence.Due to the limitations of the Lawyers.com Forums, Alan Lee, Esq.'s (the "Firm") participation in responding to questions posted herein does not constitute legal advice, nor legal representation of the person or entity posting a question.
