You would need to request reinstatement of your F-1 status by our Designated School Official (DSO)

You need to speak with your school's DSO (Designated School Official, the person who issued you the Form I-20), and you must do it immediately - any reinstatement in your school or transfer to another school has to be done within 15 days from your termination. You might have heard that you have 60-days grace period; it does not apply if you have not completed the course of study in which you were enrolled. If you do not take care of your status within 15 days from your termination you will have to leave the U.S.

You must file an application for reinstatement of your student status, Form I-539, plus supporting documentation with USCIS. You may contact the designated school official for assistance or an immigration attorney if the school does not have experience in the matter.

Speak with your DSO. Also, you might want to consult with an attorney prior to that meeting.
