How do I make sure that I qualify for DACA?

More details to this question:
In 1999 when I was 10, I came with visitor’s visa. I went to school here for 3 years. Then I left with my family to Canada in 2002 and came back in 2004 when I was 16 and 2 months. We requested asylum in the border and were allowed to enter US. The asylum was denied in 2007. I read that I do qualify because I came here before 16th and have live here continuously since 2004 and graduated from high school in 2007. What should I answer on question 12 of the I-765 form "date of last entry" without been disqualified? Should I say 1999 since my passport is stamped? or should I say 2004 which is when we returned to the US? Thank you very much!
