Today I am with company X Two companies applied for H1b transfer and both got approved: X to Y X to Z I joined Y and after a month or two, realize that it’s a mis-fit. Can I join Z directly as I already have its approval? Or should Z refile H1b transfer from Y as the previous approval is from X?

That is a good question for which I do not have an answer. Perhaps a solution is for you to leave the US and reenter the country using the approval of the company for which you intend to work. In most cases, this would involve arranging an interview at the US consulate or embassy overseas to be visaed under the desired company.
