Two diffrent Alien Numbers?

More details to this question:
I applied for deferred action back in 2012 and got Denied due to the fact that I did not have my High school diploma and I was assigned a Alien Number. My question is now that I'm applying for a I-94 so I can get my green card thru AOS via my wife do I provide them with this new Alien number that I have or leave it blank on the 102 form since I don't remember my original number on my Visa that I lost with my Passport? I came to this country legally back in 1995 with a Visa I was born in 1991 so I was young. Will there be any problems?

Hello. Yes, this A# will be on all future applications. You should provide it. You cannot file the AOS without the proof of your entry to the US.  Try and provide as much documentation or information for your entry date.
