What am I supposed to do after losing my I-94?

More details to this question:
I lost my passport with I-94. I already have my temporary passport but I do not know what to do with the I-94. Can I leave from the USA without I-94?

Yes you can leave but you can request a duplicate using the form I-102.

You can either file to replace the I-94 or, if you simply plan to depart, do so without the I-94.

You should file an application for a replacement I-94, but without knowing your current status, I can't advise on your travel plans.

You can apply for a replacement card on form I-102.
