You would get the record from the court where you got the conviction.

Go to the courthouse where your case was heard. Ask the clerk in the criminal division of the Clerk's Office for a verified copy of your docket sheet. All the information that immigration will need will be in the docket sheet.

You will need to contact the police department that arrested you to get certified copies of the arrest/incident report. And, you will also need to contact the courthouse that adjudicated your case to get certified copies of the court disposition.

The court where you went for the hearing on your DUI and where you were convicted. Contact the clerk's office of that court to start with.
