Can a company base a performance review on religion?

More details to this question:
The company my wife works for has handed out performance reviews to it's employees. In one section there is a performance question "Serves God by serving others?" She is worried because she doesn't have the same beliefs as her company. She also wants to know if it violates any laws.

If the employer has more than 15 employees, it is prohibited from discriminating against employees on the basis of their religion (or lack thereof).  However until there is an actual loss of employment or pay, or promotion or some other tangible benefit of employment, there is little the law could do on the basis of a bad evaluation alone.
