need employment contract

More details to this question:
I run an insurance agency, and I need an employment contract that can state how many minimum years my employee needs to work with me, if not, then I'd like compensation/fine a certain amount if he/she does not work a minimum of x number of years (To prevent employees from working short term and quit, and I've provided lots of resources and time on training); also need 2 months of notice if he/she quits; if violated, will also fine for that ... need the contract to state salary, benefits, vacation, sick days, bonuses, potential commission, and NDA, and non compete...what is the rate for this contract to be drafted?
