More details to this question:
I worked for an employer in NJ who has more than 1000 employees I went out on short term disability in august 2013 on family medical leave my employer terminated me because my doctor did not release me for work at the end of the 12 week leave they terminated me on Dec 3 2013, made it retroactive to Nov 4 and cancelled my health insurance on Dec 13, 2013 as of Nov 30, 2013. They have not provided the cobra informaton which I need to elect as I have serious medical issues. Did my ex employer fail to comply with cobra laws?

you need to work closely with the HR people at your former employer to get the required COBRA documentation. if the HR people are not responsive, you need to document in writing to the employer the specific failures. the key actions for you are 'notice' and 'documentation'. ed dimon, esq.
