How should I file an unemployment insurance appeal for constructive discharge?

I am going to trial at 11:30 March 2014 to argue constructive discharge. See below, I am appealing my non eligibility for my claim against Corrections Corp of America. I wish to add additional facts and circumstances. First off, I was working multiple shifts of over 12 hours straight supervising felons in a highly dangerous work environment which included frequent assaults. Assaults on staff had increased considerably during the short staffing of the prison and were at an all time high. I say this to make the point that I was working without breaks and without access to wholesome food continuously for more than ½ a day, day after day in order to satisfy the condition of my employment with Correction Corporation of America. Working 32 out of 48 hours was not uncommon. I was working hungry and tired among 250 prisoners in an open dorm setting, many of whom would have been very entertained should any violence befall me. Previously I had been employed in another part of the facility, where I could get a break and a bite to eat for just under two years. The inmates were less prone to violence and there were more staff in the vicinity. This is my first justification for my separation, for my own safety due to the conditions imposed on me by the reality of being surrounded constantly by felons and having to maintain my safety and that of others on little sleep or nourishment. The change in my working conditions changed drastically when I cooperated in the compliance review ordered by the Federal judge for Correction Corporation of America under staffing. International Criminal Court was told by a warden that I had been “talking trash to people and I knew who” and I am immediately reassigned where they wanted me to be. The reassignment carried a menacing tone with it. I had never missed a day of work (04:30 start), never been late for work, been written up once for a minor misunderstanding, been safety employee of the month in 2013, in other words I was a good, conciseness employee until I was singled out to be punished for.

Constructive discharge is often defined by someone voluntarily terminating their employment for a reason that any "reasonable" person would terminate their employment.
