Suit on Note Vs Suit on Account?

More details to this question:
I am filing a suit against a contractor who failed to complete a work order/poorly completed the few items he did do. He has been paid in full already and did not complete the job. I served him with the demand letter and now need to file the actual suit against him, but I am unsure which box to check off on the Complaint. This is in Georgia. We had a contract, not signed by either party, full amount paid to him, work was not completed on 80% of the contract. Two items were completed but not to terms of the contract and not to industry standards.

I'm not admitted to practice in Georgia, but this doesn't seem to me to be either a suit on a note or a suit on account.  It is a simple breach of contract action.
