Who makes the decision regarding my friend's 18-year-old son?

More details to this question:
My friend’s 18-year-old son came to live with me last year because of the violence in Philly. 2/24/13 he was in a car accident, concussion and severed spine. Unable to make decision at the moment. My insurance is covering him and I signed all paperwork. Mom shows up and says I have no rights to say what rehab he goes to, who is correct.

She is unless you can get a court to appoint you as guardian which will be tough unless mom is totally nuts.
