can I make a report without harming my professional license

More details to this question:
I am a license massage therapist who was working for a company. This company has been lying about treating patients and have some workers who aren't license to perform these duties. Patients will just come in and sign a date sheet a leave. The owners who is a physically therapist lies on soap notes saying he treated the patient and then bills it to insurance companies and they have a o.d doctor saying these patients who were in accidents need surgery. We also have a massage therapist who is a partnership in this clinic and she doesn't work there any more, and the clinic is using her license without her supervising. The reason why I'm asking my question is because when my boss left the job the owner or the business keeps making me finish up her soap notes, when I decline writing them they said I'm the only massage therapist who is able to write it and ill will be fine. If I report this clinic will I lose my license for writing soap notes. Because I just quit my job asap once I knew it
