QUESTION
I need to know what qualifies as medical malpractice...

More details to this question:
I am currently switching doctors for the simple fact that my doctor is not treating me correctly, when I told her I was going to switch to another doctor she told me that she has to "console" with my new doctor before switching because I failed one UA in the 4 years I have been seeing this doctor.. She is also taking me off a medication that I need even if it is classified as a narcotic I have chronic pain including scoliosis, and a bister disease. I have been on this medication for 18 years. I need to know if what she is doing is okay? or what I can do about this issue.

Medical malpractice involves medical care that is below accepted standards which has a negative consequence on a patient's health. None of the things that you describe sound like medical malpractice to me.
