Release from hospital subsequent death

More details to this question:
My wife went into emergency from her visit to her personal practicioner. Soon they found liver disease she stayed hospitalized from March 8th until March 14th they released her she had a nurse visit and was told to visit her doctor on Monday March 18th he sent her back to emergency imedaitly. They then fouund some type of infection but never really heard what it was things starteing getting worse quickly over night march 19th they discovered a heart attack from thier it went to life support machines. The family removed her from teh life support machines march 21 2:15 pm she passed away. My bigest question is hoe could she be relaesed on March 18th? Should she not have remained under care?

This may be medical malpractice regarding delay in diagnosis of heart attack and unsafe discharge.
