Trauma Center, ER care

More details to this question:
35yo male multiple blunt chest trauma from motorcycle accident. Doctors claimed pt with gcs 15, bilateral pneumothorax with bilateral chest tubes inserted, flail chest, cardiac tamponade, return of circulation, then stabilized & sent to icu for 3 days. Pt then dies on third day. How was there cardiac tamponade, but no thoracotomy done or noted needle aspiration noted on pt chest? and why did they state that pt was coding for 59mins? What took them so long to resuscitate if it was a tamponade? If chest tube was inserted in ER, how come bilateral lungs were completely collasped? Were they inserted to the right place? Note, the hospital is a Level 2 Trauma center. What do i need to do to get answers?
