What do I need to file a medical malpractice suit?

More details to this question:
I was transported from on hospital to another hospital for emergency surgery. I waited over 7 hours after arriving for the surgery. While waiting for surgery, my appendix ruptured. This resulted in a 5 day ICU hospital stay and loss waiges.
