What type of Attorney do I need?

More details to this question:
My primary physician was changed. I hadn't met the new doctor yet, but she requested an urine (drug screening) test under false reasons. Twice the urine test came back Positive for Cocaine. I have never used Cocaine/Crack in my life. I strongly requested a blood test, which came back Negative for Cocaine. But on my Health Care Issues, it still states Addiction to Cocaine. I want it removed.

I would just change doctors and not refer to the one that you discuss in this post when reporting your medical history.
