Medical records are not legal documents. They are used as exhibits in civil/criminal cases and cannot be admitted without complying to Rules of Evidence.

If they are verified by a records custodian, then yes. They can also be certified.

They can be, but in order to be viewed by the court as evidence, they need to be authenticated. If both sides agree that they are business records, that would suffice.
