When a settlement is complete who is the responsible party for the medical bills once the attorney is paid their 1/3 %.Does the attorney have a right to promise to pay my medical expenses or do I the client handle it.The contract I signed is what I agreed to.My attorney said I have a medical lien on my case and they signed promissory note that the doctors get paid even before I do....I need help I've been trying to drop this attorney but she tells me I can't due to the fact the law firm has a lien on me????

It sounds like your attorney issued what is known as a "letter of protection" to the medical providers so you could get the medical treatment you needed to deal with your injuries without paying for the services out of your pocket.
Without the attorney promising to pay the medical providers out of the settlement, it is very unlikely that any of the doctors would have treated you, unless you paid the providers upfront.
