How to settle a personal injury claim

More details to this question:
Fell in a clothing store due to a clothes rack left unattended. 20ft wide X 7ft high with a cross bar approximately 10 inches off the floor. Tripped by the bar upon turning around. Appeared to be a clear opening in to another area of the store. Fell and hit my head and injured by shin. Ambulance called after someone said to call 911. No assistance from the store. Expenses being paid by my medicare. Store insurance says they are not responsible, my primary care insurance is. Outstanding expenses not paid by insurance - ambulance, travel, pain and suffering.
