If I got a lawyer I probably couldn't get much more than that after all the Dr. bills and Lawyers fee's right? Even if a lawyer could get me 3K or so more, It really wouldn't be worth all the trouble and waiting time would it?

You are entitled to reimbursement for your medical expenses, lost wages, and pain and suffering. Shoulder surgeries have a difficult recovery, and the pain and suffering alone likely far exceeds the value of their offer. Based on your brief description, you would be making a costly mistake if you accepted the insuance company's offer.
