Can I qualify for mental/ physical Disability? What are my rights?

More details to this question:
My employer had prior documentation on file from doctor's to my employer months before termination my primary care doctor stating I was under continuing doctors care undergoing treatment and continuing medication changes under further notice. Also on a write up by the employer I was told to sign a write up to agree to see the employers EAP behavior health specialist if I didn't sign or agree I would be terminated. I did comply with and was continuing visits before and after termination along with my primary care doctor visits. The EAP doctor met with andcontacted my employer several times over my mental health condition that I was undergoing treatment plans for several different mental, hormonal, and chemical imbalances stating not a quick fix / will take time with continuing doctors care. A few weeks later I was terminated with a severance package that ends 10/15/19

Only the Social Security Administration can determine whether you qualify, based on a functional analysis.  You can apply on your own at www.ssa.gov.  Please be aware that the process could take a good six months, longer if you must appeal.
