Do I first apply with Social Security and wait to get an attorney if denied?

More details to this question:
Applied 6 years ago, went through several denials, let the last appeal period lapse. This is for nephew who is diagnosed ADHD, but it most likely also bipolar. Recently tried to work after his mother died, has lost 3 jobs. Can't seem to retain instructions for very long, has constant need for reassurance.
