who can i talk to about my case that's five years old?

i fell on a side walk in the district of columbia in the rain and broke and dislocated my arm and elbow had to have surgery to reconstruct and repair everything also had some nerve damage have not been back to work since over this time period i have developed other illness, i have hypertension, I'm a type two diabetic i take insulin 4 times a day, i have restless leg syndome, were my legs hurt at all time mainly at night or if i stand of two long, and list goes on
