As an employer, what forms do I need to have an illegal with a pin number fill out for withholding taxes?

If the person is illegal, you should not hire the person. You should contact Immigration and Customs Enforcement.

As an employer, you need to complete an I-9 for each employee. The I-9 requires the employer obtain proof that the employee is a citizen or otherwise legally in this country. It is unlawful for an employer to not obtain and retain an I-9 for each employee.
