Foreign Bank and Financial Accounts

More details to this question:
I an a resident alien for tax purposes living in US since 2007. My account who files for my taxes never told about FBAR rule. I came know about this only this year and I am filing form TD F 90-22.1 for the first time. Though the amount is too small for last few years, can I get a penalty from IRS for not filing. Can I hold my accountant responsible because he filed it on my behalf and he should have told me about this.
